Please be sure to have the following scanned and uploaded to your client portal:

 

• A photo ID for yourself and/or spouse

• Social Security Cards for all family members

• All of your W-2 forms, 1099 forms, etc.

• Copy of prior-year tax return (if you're new to us)

• Bank account and routing number for Direct Deposit

• Tuition fees & expenses (Form 1098-T)

• Total paid to daycare provider and their address, phone, and EIN numbers

• ITIN numbers (If applicable)
